These Terms & Conditions ("Terms") govern your access to and use of the Website and services provided by Intention Holistic Health PLLC, a Texas Professional Limited Liability Company ("Company," "we," "our," or "us").

Intention Holistic Health PLLC is a Texas-registered Professional Limited Liability Company (PLLC).

Clinical services are provided by Alicia Harrison, MSN, APRN, FNP-C, a Family Nurse Practitioner licensed in the Commonwealth of Kentucky and state of Texas.

All business operations are governed by Texas PLLC regulations.

All clinical services are governed by Kentucky nursing law and telehealth requirements.

No clinical services are provided to Texas residents at this time.

2. Nature of Services

Intention Holistic Health PLLC offers the following services:

2.1 Clinical Nurse Practitioner Services (Kentucky Residents Only)

Clinical NP services include:

  • Wellness consultations
  • Functional lab interpretation (for education/wellness only)
  • Lifestyle, nutrition, and mineral balance recommendations
  • Telehealth wellness visits

These clinical services:

  • Are available only to individuals physically located in Kentucky at the time of service
  • Require completion of all intake forms and consent documents
  • Do not constitute primary care, urgent care, emergency care, OB-GYN care, or chronic disease treatment
  • Do not include prescribing at this time
  • Are adjunctive, educational, and supportive in nature

A patient-provider relationship is established only after:

  • Intake and consent are completed
  • Eligibility is verified
  • You are formally accepted into the clinical practice

3. No Medical Advice

Website content, blog posts, social media, emails, guides, and educational materials are for general informational purposes only.

They do not:

  • Provide medical advice
  • Replace the care of licensed medical professionals
  • Establish a patient-provider relationship

Always consult your primary care provider, OB-GYN, or specialist for medical evaluation and treatment.

4. Telehealth Policy

Clinical telehealth services may only be provided to users who:

  • Are official residents of Kentucky, AND
  • Are physically located in Kentucky at the time of the appointment.

You must truthfully disclose your physical location. Services may be refused or terminated if this information is inaccurate.

The risks of telehealth — including internet interruptions, privacy risks, and technology limitations — are acknowledged by the user.

5. Functional Lab and HTMA Disclaimer

Functional labs—including HTMA—are provided strictly for:

  • Wellness
  • Lifestyle guidance
  • Educational purposes

These labs are:

  • Not diagnostic
  • Not intended to detect or treat disease
  • Not a substitute for conventional medical testing

Any laboratory images shown on the Website are fictional, sample-based, or de-identified.

6. Client Responsibilities

You agree to:

  • Provide accurate information
  • Participate honestly in services
  • Avoid submitting PHI through non-secure channels (email, Kajabi, Website forms, social media)
  • Maintain a primary care provider
  • Use information responsibly and within the intended scope

7. HIPAA & Privacy

7.1 PHI Collection

Protected health information (PHI) is collected only via:

  • Practice Better (HIPAA compliant)
  • Approved HIPAA telehealth platforms

PHI is not accepted via:

  • Email
  • Website forms
  • Kajabi
  • Text
  • Social media messaging

7.2 HIPAA Notice

Clinical patients receive a full HIPAA Notice of Privacy Practices.

This Website itself is not a HIPAA-covered platform.

8. Payment, Cancellations & Refunds

  • Payment is due at time of service or purchase
  • Insurance is not accepted
  • All digital products and completed services are non-refundable
  • Cancellation/no-show policies may apply to clinical appointments

9. Intellectual Property

All content, including written materials, images, graphics, PDFs, courses, and downloads, is owned by Intention Holistic Health PLLC.

You may not copy, reproduce, distribute, or repackage content without written consent.

10. Photo & Media Disclosure

  • Some photos are by Amber Tice Photography and used with permission
  • Other images are licensed stock photos or illustrative graphics
  • No photos depict actual patients or clinical encounters
  • All lab screenshots or diagrams shown are fictional or de-identified examples

11. Limitation of Liability

To the maximum extent permitted by law, Intention Holistic Health PLLC is not liable for:

  • Results from coaching or wellness recommendations
  • Technical failures of telehealth
  • Inaccurate information provided by users
