  • Individuals who are physically located in Kentucky during telehealth visits
  • Individuals who have completed all required intake forms and consent documents

Services may be refused or terminated if location requirements are not met.

3. Clinical Services and Patient-Provider Relationship

Clinical services provided by Intention Holistic Health PLLC are delivered by a Kentucky-licensed Advanced Practice Registered Nurse (APRN) and Family Nurse Practitioner (FNP-C) in accordance with Kentucky Board of Nursing guidelines.

A patient-provider relationship is established only when:

  • The client has completed all required intake forms and consent documents
  • The provider has agreed to accept the client for care
  • An initial consultation or visit has been conducted

4. Adjunctive Care

Services provided by Intention Holistic Health PLLC are intended to complement—not replace—your existing healthcare team.

Our services are designed to work alongside:

  • Other specialist care as applicable

We do not prescribe medications or manage chronic prescription drug therapies. Clients should maintain relationships with their primary healthcare providers for comprehensive medical management, emergencies, and pharmaceutical needs.

5. Functional Lab Testing Disclaimer

Functional labs, including HTMA (Hair Tissue Mineral Analysis), offered or interpreted by Intention Holistic Health PLLC are:

  • Not diagnostic
  • Not intended to detect, treat, or cure disease
